SMILE FAQs

SMILE is shorthand for “Small Incision Lenticular Extraction”. ReLex is shorthand for “Refractive Lenticular Extraction”. Both of these names refer to the small lens, or lenticule, which is removed from the central part of the cornea.

PRK is also flapless, but the two procedures work very differently. PRK removes the surface layer of the eye and treats the tissue directly underneath, which is why recovery involves more discomfort and a longer vision timeline. SMILE works entirely within the interior of the cornea so the surface is never opened. SMILE shares PRK’s no-flap benefit while delivering recovery much closer to the LASIK experience.

Most healthy adults with a stable prescription are strong candidates for SMILE. You may qualify if you:

  • Are at least 22 years old
  • Have had a stable prescription for at least two years
  • Are nearsighted, with or without astigmatism
  • Have sufficient corneal thickness
  • Not pregnant or nursing

SMILE cannot correct farsightedness. SMILE is FDA-approved only for nearsightedness and astigmatism. Patients with farsightedness are better suited for LASIK or PRK, both of which Dr. Caster also performs! Your personal consultation will identify the most appropriate procedure for your situation.

SMILE can treat a meaningful range of nearsightedness and astigmatism. As a general rule, patients with higher levels of correction tend to have slightly less predictable outcomes than those with milder prescriptions, and this applies to all laser vision correction techniques. During your consultation, you will get a better picture of expected accuracy based on your specific prescription.

Yes, SMILE is performed while you are awake, but numbing drops are applied beforehand so you will not feel anything during the procedure. Most patients are surprised by how quick and comfortable the experience is from start to finish.

SMILE is designed as a one-time procedure, and the vast majority of patients achieve their desired results after a single treatment. In the rare case where a prescription shift occurs or a fine-tuning enhancement is needed, it is performed using LASIK or PRK rather than a repeat SMILE procedure. This is because the corneal tissue treated during the original surgery cannot be reshaped the same way a second time. Candidacy for an enhancement depends on whether sufficient corneal thickness remains, which will be evaluated before making any recommendations!

Yes, SMILE is FDA-approved to treat astigmatism even when nearsightedness is not a significant factor! Astigmatism occurs when the cornea has an irregular curvature, causing blurred or distorted vision at all distances. SMILE corrects this by precisely reshaping the corneal tissue to restore a more uniform curvature. If you have been told in the past that your astigmatism made you a poor candidate for laser vision correction, it is worth discussing your specific measurements during the consultation, as SMILE may still be a viable option depending on the degree and type of astigmatism present.
